You can file for your application via MISMO. All applications in regards to theoretical and practical exam can be done in this platform.

Practical Assessment
As of 2023, passers of theoretical examination will get an automatic practical assessment application number. Hence, there is no need to apply for practical assessment permit on MISMO.
Only those who did not use their license after 5 years will have to get the practical assessment permit. Practical assessment is one requirement for COC revalidation for those who weren’t able to use their COC.
